What's inside

Twelve pages designed to find your patterns.

Five minutes a day. Real data your practitioner can act on. The same tool we use with chronic pain patients in clinic.

01 · Daily

Daily check-in

A 60-second log of pain, sleep, mood and energy. Builds the data picture without becoming a chore.

02 · Visual

Body pain map

A clear front-and-back diagram. Mark where it hurts, what type of pain, how it changes. No guessing.

03 · Insight

Trigger tracker

Side-by-side cards for what made it worse and what helped. The patterns you've been missing become obvious in a week.

04 · Review

Weekly review

A guided look back at your week — best day, hardest day, patterns, questions for your practitioner. Real data beats trying to remember.

About this diary

Built in clinic. Used in clinic.

We're an osteopathy and rehabilitation clinic in Newcastle. Our team treats everyone from elite athletes recovering from ACL surgery to people who've been living with chronic back pain for a decade.

This is the same diary we hand to chronic pain and post-surgical patients at intake. It's not a marketing freebie that looks pretty and says nothing — it's a clinical tool that turns vague "I think it's worse on Tuesdays" into hard data your practitioner can actually work with.

Use it on your own, share it with your GP, or bring it to your first session with us. We made it free because pain is hard enough without paywalls in the way.
